Welcome to Our Office!We are glad that you dropped by our web site and are thinking of becoming a new patient. There are a couple of things to consider before calling for an appointment.Can We Help You?Our doctors are well-trained and experienced, with a total of over 100 years of clinical experience here in the office. Our days are filled with anxiety, depression, post-traumatic stress, mood swings, panic and phobias. We treat all the common disorders and most of the uncommon ones as well.Or Maybe Not...Our goal is to help our patients feel better and function better. However, we are interested in treating patients, not in just evaluating them. If you need an evaluation for disability, a legal problem or a custody battle, we are not going to be very helpful. We are also not equipped or inclined to treat patients for perpetrating child abuse or domestic violence. InsuranceIt helps if you have it. We work with most of the companies that are common in the Mid-Willamette region. One of our receptionists will take your name and number and ask you a few general questions. Give us a call Monday through Friday between 9 and 3.After Scheduling Your First AppointementTell Us About YourselfWe ask that you fill out our intake form before coming in for your first appointment. This covers the standard medical areas such as allergies, surgeries, and past illnesses. It also asks some general questions about children, parents, school, work and relationships. You may find that spending a few minutes writing down your past experiences gives you a little different perspective on your present situation. We will send you an intake form after scheduling your initial visit, or you can download one and print it out.You may download a pdf file of our intake form by clinking here. To download and print pdf (portable document format) files, you need to have Adobe Acrobat or Adobe Reader installed on your computer. If you don't have this already, you can download it from Adobe at no cost. Be PunctualThere will be a couple of forms to read and sign when you arrive for your first appointment, and we will need to take a copy of your insurance card. Depending on how many other patients we are serving and how fast you deal with paperwork, this can take from five to fifteen minutes.Be sure you have your intake form completed before you arrive for your appointment. Don't count on finishing it while you wait for your doctor. They are often right on schedule and rarely more than five minutes behind. If you arrive late for your appointment, you are shortening the time that your doctor has to understand your difficulties and plan how to help you.
