Reminders

  • If your insurance plan requires it, please obtain pre-authorization before your first visit.
  • The co-pay printed on your insurance card might not be the correct one for mental health services. Unfortunately, mental health treatment coverage is not regarded as general health care.
  • Though we try to verify your benefits before your first visit, be aware that information about your coverage is not a guarantee of payment.
  • Most plans have only paid for a limited number of outpatient visits in the past. This may improve with the recent (2005) passage by the Oregon legislature of a law requiring parity mental health treatment.

Participating Provider

Salem Psychiatric Associates is a participating provider for several insurance companies. They include:

  • CNA
  • Health Care Direct
  • Lifewise
  • ODS
  • OneHealth Plan
  • Pacificare
  • PacificSource
  • Providence
  • Providence Preferred
  • Qual-Med
  • Regence Blue Cross
  • Regence HMO

Being a participating provider with these companies means that we have contracted with them and have agreed to write off any amount above their allowed charges. You are still responsible for your co-payment.

Medicare

Salem Psychiatric Associates is not presently a Medicare participating provider. This means that you are responsible for your bill. Medicare will reimbures you for a portion of the charges.

Beginning in November 2005, we will become a participating provider. This means that Medicare will send payment to us and we will accept Medicare reimbursement as full payment for your bill.

Oregon Health Plan and Oregon Medical Assistance Plan

Salem Psychiatric Associates is not an OMAP (Oregon Medical Assistance Plan), OHP (Oregon Health Plan) or BCN (Behavioral Healthcare Network) provider. If your medical insurance is one of these, please click on this link to Valley Mental Health, which does participate in the Oregon Medicaid program and accepts both open cards and BCN.

(Our doctors do treat a few OMAP and OHP patients through their contractual arrangement with Valley Mental Health, so if you are already a patient at SPA and have one of these insurances, you can ignore the preceding paragraph.)

Workers Compensation

We regret that we are unable to treat patients for illnesses about which there is a worker's compensation claims pending.
